At-Home Fibromyalgia Questionnaire

Take this assessment to see if you should talk to your doctor about fibromyalgia.

Could your exhaustion, pain, or brain fog be fibromyalgia? There is no simple blood test for this condition, but this short questionnaire can help you show your doctor how widespread your physical pain is to help him or her figure out if it's a bigger problem than localized pain. Print out the questionnaire and mark the areas in which you've had pain in the past three months. Then, check your overlapping symptoms and add up the numbers. If your score is seven or higher, bring this sheet to the doctor with you to discuss fibromyalgia.
